3. Form/document name: a. Self-assessment questionnaire	a. Download
Form/document purpose: Self-assessment of technical capability and quality management requirements in order to authorize packers. Additional information: Complete by hand and tick the options applicable to the company. Methods of sending: Email or post Requires registration: NO

The forms are printed on the printer, filled in and then sent by fax to no. +4-021-350 82 88 or they are scanned and sent by e-mail to the address dcp@rarom.ro, together with the other documents specified in the List of documents requested for authorization.

The completed forms can also be brought in person to the PCD offices.
