Useful information regarding Type Approval and Products Certification activities

Form/document purpose: Applicant and product identification, applicant declaration, establishment of nominated person for the relationship with PCD, official notification to RAR regarding the intention to certify the products. Additional information: – Point 2.1: CERTIFICATION SYSTEM and CERTIFICATION SCHEME – see CHAP. 6 OF RG-00 Methods of sending: Fax/email (pdf) Requires registration: YES
2. Form/document name: a. List of documents required for certification	a. Download
Form/document purpose: Establishing the documents to be submitted to the file. Additional information: – The certification scheme chosen when completing the certification application is used; – On the column corresponding to the number of the certification scheme, the documents to be submitted to the file are identified. Methods of sending: Email or post Requires registration: NO
3. Form/document name: a. The self-assessment questionnaire	a. Download
Form/document purpose: Self-assessment of technical capability and quality management requirements for certification. Additional information: – Complete by hand and tick the options applicable to the company; – The annex with the organization chart of the company must be attached to the form. Methods of sending: Email or post Requires registration: NO

The forms are printed on the printer, filled in and then sent by fax to no. +4-021-350 82 88 or they are scanned and sent by e-mail to the address dcp@rarom.ro, together with the other documents specified in the List of documents requested for authorization.

The completed forms can also be brought in person to the PCD offices.
